What is your name?
Nicole Szpilka
How many kids do you each have and what are their ages?
My husband and I have two children—a 3 year old and a 2 year old.
What is your favorite thing about the Sayville/Patchogue area?
We love the incredible sense of community here. There are so many amazing local restaurants and small businesses to support, and you can’t beat spending time on the Great South Bay. It’s the best place to live, work, and raise our family.
Tell us about your business—name and services offered!
My husband and I founded Coastal MD, an integrative wellness practice, five years ago with the vision of providing personalized, proactive healthcare. We started with one small office that was open just three days a week. Today, Coastal MD has grown to three locations throughout Long Island, an increasing telemedicine platform out of state, primary care, women’s health services, and collaborations with both plastic surgery and general surgery. We are also proud to have grown our team to more than 40 employees.
Before starting Coastal MD, I worked as a physician assistant in surgery. Today, I serve as Vice President, while continuing to care for patients clinically, allowing me to stay connected to the people who inspired us to build this practice.
My mission has always been simple: help as many people as possible achieve their health goals through a personalized approach. We focus on increasing our patients’ health span through personalized care, frequent follow-up, and ongoing motivation. We offer comprehensive consultations, advanced bloodwork panels and review, male and female hormone replacement therapy, nutritional counseling, IV vitamin hydration, non-narcotic pain management, medical massage, peptide therapy, stem cell therapy, aesthetic treatments, and more—all designed to help our patients live healthier and happier lives.

What inspired you to start your business?
After years in traditional medicine, I realized that many patients needed more time, more education, and more support than the healthcare system could provide. COVID also highlighted this. My husband and I wanted to create a practice where we could truly partner with our patients, helping them not only treat illness but optimize their health and prevent disease. Our goal has always been to help as many people as possible become the healthiest version of themselves.
What do you love about your business/favorite part of what you do?
The most rewarding part is seeing our patients transform their lives. Whether someone has more energy, balances their hormones, loses weight, overcomes chronic pain, or simply feels like themselves again, it’s incredibly fulfilling to be part of that journey. I also love mentoring our growing team and watching our practice continue to evolve while staying true to our mission.
How has motherhood influenced your business journey?
Motherhood has been one of the most rewarding and humbling chapters of my life. Balancing the demands of growing a business while raising two young children has taught me that perfection isn’t the goal—being present is. It’s shifted my perspective on success, making me far more intentional with my time and placing a greater emphasis on the quality of the moments I spend with the people who matter most to me.
Motherhood has also taught me the importance of leaning on others, trusting my team, asking for help when I need it- it truly takes a village. Those lessons have made me not only a better mother, but a stronger leader.
More than anything, becoming a mom has deepened my passion for health and wellness, especially in the women’s health space. It has reinforced my commitment to building a life, and helping others build their lives that are healthy, meaningful, and fulfilling. The greatest success is creating a life that allows you to truly be present for the people you love.
What makes your business unique?
Our approach is truly personalized. We don’t believe in one-size-fits-all healthcare. We take the time to understand each patient’s goals, create individualized treatment plans, and see them regularly to provide accountability, education, and encouragement. We aren’t just treating symptoms—we’re helping people increase their vitality and improve their quality of life. At Coastal MD, increasing vitality isn’t just our goal—it’s our mindset.
What advice would you give to another mom?
Give yourself grace. There will never be a perfect balance, and every season of life looks a little different. Don’t be afraid to ask for help, trust your instincts, and remember that your children don’t need perfection—they just need you.
What is the best piece of advice you’ve gotten from another mom?
“It goes too fast. Enjoy every second—even the crazy and chaotic ones.” One day you’ll look back, smile, and realize those were some of the most beautiful parts of the journey. They are right!
Anything else you’d like to share?
I’m incredibly grateful for the support of my family, my husband, our amazing team, and the community that has trusted us with their care. Looking back at where we started—with one small office open three days a week—it’s amazing to see how far we’ve come. I’m excited for what’s ahead as we continue helping more people live healthier, longer, and more vibrant lives while raising our own family in the community we love.
